Healthy young men have leaner bodies than women. Men’s bodies also show increased muscle definition. The most frequently treated areas in men are:

  • Neck and chin
  • Chest
  • Waist

Liposuction in men is also the treatment of choice for most gynecomastia (male breast enlargement).

Creating a six-pack.  Male patients frequently ask for abdominal “liposculpting” or abdominal “etching.” Some patients also ask for “high definition liposculpture.” All of these terms refer to selective fat removal on the front of the tummy to accentuate the muscles and give a “six-pack” appearance.

In “high definition liposculpture,” fat is removed from the entire front of the tummy and from the waistline out to the side and around to the back. Then extra fat is removed from near the borders of the abdominal muscles to emphasize bulk and strength in the rectus abdominis muscles and the “obliques.” Results can be impressive, particularly if the man already has well developed muscles; but, even lightly muscled men get improvement and increased definition with selective fat removal.

Although certain specific instrumentation and devices have been promoted as essential to creating the “six-pack,” highly defined musculature is achievable with a wide variety of instrumentation.  The most important determinant of the quality of results is the skill and experience of the surgeon.

Gynecomastia refers to development of female-like breasts in a male.

Adolescents. The most frequent cause of gynecomastia in teenagers is hyper-development of breast glandular tissue in response to rapidly changing hormone levels. Most breast enlargement in boys is temporary and resolves spontaneously as hormone levels subside to normal. If the enlargement persists for more than two years, surgery may be required to remove the gland. Liposuction will be beneficial, if a significant part of the enlargement is due to fat.

Adults. Most breast enlargement in adult men is due to excess fat. The treatment of choice is liposuction. Other causes of breast enlargement, such as drugs, alcoholism, or use of steroids, should be ruled out before treatment.

In both teenagers and adults, a careful history and exam should be done before treatment of gynecomastia.
